Help Us Help You by Keeping a Medication List

You can be a partner with San Juan Regional Medical Center in your healthcare. We are asking everyone to keep a complete list of all their medications, prescriptions and supplements and to bring this list with them when they come to the hospital for treatment. To make this task easier, SJRMC has created a handy medication packet that includes a medication list and a magnetized refrigerator bag to keep it in. These kits are available for free at a variety of locations throughout San Juan County, including pharmacies, senior centers and at the front desk at the main hospital facility, 801 W. Maple St.

The medication list is part of our medication reconciliation policy which was implemented early this year. Medication reconciliation is the process that compares a patient’s best known list of current medications, including over the counter and herbal medicines, against the admission, change in level of care and discharge orders.

Discrepancies are brought to the attention of the physician and, if appropriate, changes are made to the medication orders. The purpose of medication reconciliation is to obtain and document a complete list of patients’ medications in order to maintain the proper continuity of care.

The medication form is intended to help patients and family members remember all of the medicines they are taking, and they can keep the form in the magnetized bag. The packet may be placed on a patient’s refrigerator door or in their car. It will then be available to first responders in the case of emergency.

The form should be taken to all doctor visits and all hospital visits. All changes to medication should also be written on it. Please see the back of the form for complete instructions on its use.

This new medication reconciliation policy is another step in SJRMC’s efforts to increase patient safety.
